Diabetes Weight Management Service

The new NHS Digital Weight Management Programme (DWMP) offers remote access to weight management services for those with obesity plus diabetes and/or hypertension. With three levels of support and a choice of providers, it is designed to offer service users a personalised level of intervention to support them to manage their weight, improve quality of life and improve longer health outcomes.

The new service will work alongside, and not replace existing weight management services funded by Local Authorities.

The programme aims to offer remote access to weight management services, including three levels of support, to a large population with obesity and diabetes and/ or hypertension, in order to support service users to manage or reduce their weight, and improve longer term health outcomes.

The three levels of support aim to reduce health inequalities by providing additional coaching for groups who are less likely to complete behavioural and lifestyle programmes.

Level 1: access to digital content only
Level 2: access to digital content plus 50 minutes of human coaching over the 12 weeks
Level 3: access to digital content plus 100 minutes of human coaching over the 12 weeks

REFERRALS

Searches, referral forms and protocols have been centrally disseminated through resource publisher and should be available for use in both EMIS and SystemOne.

Referrals can be made by any member of staff working in General Practice, not just GPs. All referrals need to go through ERS

ELIGIBILITY CRITERIA

Inclusion Criteria:

  • Age 18+
  • BMI of 30+ (or >27 for BAME groups) with a diagnosis of diabetes and/or hypertension or both.
  • Exclusion Criteria:

An individual who meets any one or more of the following exclusion criteria is not eligible to access the service and will not be eligible for referral:

  • Severe/moderate frailty as recorded on a frailty register
  • Is pregnant
  • Has had active eating disorder
  • Has had a bariatric surgery in the last two years
  • People for whom weight management programme is considered to pose a greater risk of harm then benefit
  • For those individuals aged over 80, the referrer will need to provide supporting information that they are suitable for the programme.
