Patient's Right to Share:
If your patients do not wish for their information to be shared via the Whole Systems Integrated Care tools, they can complete the form below to opt out of information sharing and return it to their GP.

Care Professional users with access to the WSIC Dashboards can access the tools by clicking on this link

WSIC Request for Change (RFC) Form
The request for change (RFC) form can be used to request new dashboard, enhancements to dashboards or new data feeds.
The RFC should be filled in and emailed back to the WSIC Dashboard team at nwlccgs.wsic.dashboards@nhs.net
The WSIC team will send an acknowledgement email to the requestor with the confirmation of the next steps
